Job Description Responsibilities:

Provide comprehensive nursing care to patients in a home care setting.
Manage and oversee home care cases, ensuring quality care and support.
Collaborate with healthcare professionals and families to develop and implement care plans.
Monitor patient progress and adjust care as needed.
Document patient information and progress in a timely and accurate manner.
Qualifications:

7 months to less than 1 year of experience
college/CEGEP
Experience in home care or nursing care management is preferred.
Strong communication skills and ability to work independently.
Ability to adapt to flexible working hours, including early mornings, evenings, and some mornings to be scheduled.
